This packet must be filled out completely to participate in the 2010-2011 school year.  Please remember to circle all sports that your student is considering trying out for during the entire school year.

Packets can be turned into the athletic office all summer long.  Do not wait until the last minute and jeopardize being able cleared when tryouts begin.
